Middletown has, what?--two stop lights? ;-) Actually, what I remember from my visit there is there was only one road in and out. I can imagine it gets pretty congested at certain times of the day, hence the camera.
There might be three, depending on exactly where the western city limit line is. :) This cam is at the main intersection; it's a US highway and a state highway and sometimes there is actually some traffic there. I'll have to dig around to see if I can find a link to that camera.

My guess is it is a sensor for the fire station. It turns all of the lights red for the safety of the fire trucks. No camera's involved?
Looks big for a sensor but I guess it could be. It's only a few hundred feet from the fire station.
